VEHICLE EMISSION RECALL - PROOF OF CORRECTION FORM COMPLETION






California residents who own an affected vehicle may be required to send in a Vehicle Emission Recall - Proof of Correction form to the California Department of Motor Vehicles (DMV) as evidence of recall completion when they apply for vehicle registration renewal. Each customer whose vehicle is registered within California must be given a completed Vehicle Emission Recall - Proof of Correction form after the recall has been completed. Packages of this form have been sent to California dealers along with this bulletin. Additional forms are available by contacting your Field Technical Managers (FTM).

Note:
This form MUST be completed by the Dealership's Service Manager using a permanent ink pen. A fine point pen is recommended. When filling in the information, copy the information directly from the vehicle, NOT the repair order. ALL INFORMATION MUST BE PRINTED AND NEAT.

DO NOT COMPLETE THE FORM UNLESS THE RECALL WORK IS COMPLETED!

1. Enter the vehicle's license number.

2. Enter "HYUNDAI"

3. Enter the vehicle's model year "1993"

4. Enter "SCOUPE"

5. Enter the Vehicle Identification Number (VIN).

Note:
1 digit per box, and the digits must be legible. (Use a fine point pen.)

6. Enter "012"

7. Enter your dealership's name.

8. Enter your dealership's physical address. (Not the mailing address.)

9. Enter date of repair.

10. Sign the form

To submit a campaign claim for the Scoupe Ignition Coil Assembly Wiring Position Modification Campaign you need to enter your information on the HMA Campaign/ValueCare Maintenance Claim Enny Screen (CCM Screen - ADP. #23 Screen Reynolds & Reynolds.) The screen you use on your DCS system may be formatted differently. but will have the same fields listed below.

Dealer code:
Each Hyundai dealership has a unique dealer code. This dealer code is coded into your DCS terminal and is supplied automatically. You do not need to. nor can you. key the dealer code.

Claim #:
A unique six (6) digit number identifying the claim. Can be alpha and/or Numeric, Example: 12345 A. It can be the repair order number or a number taken from your claim register.

Op Code:
You will use one of the following op codes depending on which modification you completed on the vehicle:

30009A2 = Inspect & Re-Route Ignition Coil Wiring
300009R1 = Inspect. Replace, & Re-Route Ignition Coil Wiring

NOTE:
Op code time is 0.5 for either modification.

The following data may be entered on the campaign claim screen up to 15 times to accommodate up to 15 of the same campaign repair op code to separate vehicles,

R.O.#:
Enter the repair order number of the VIN having the campaign performed All six digits of the screen must be filled. Any combination of letters and numbers will be accepted.

VIN:
Enter the complete 17 character vehicle identification number of the VIN having the campaign performed.

Mileage:
Enter the mileage at the time of repair of the vehicle having the campaign performed.

Repair Date:
Enter the repair date in MM DD YY (Month Month Day Day Year Year) format.
Example: October 23, 1993 = 10/23/93.

NOTE:
The claim number and op code entered on the DCS screen will repeat for all of the R.O.#'s and VIN's entered Into that particular screen for each VIN repaired, up to 15 times.
