Claim Information

Submit a Product Campaign Claim with the information indicated.


* Campaign Administrative Allowance.

** The "Parts Allowance" should be the sum total of the current GMSPO Dealer Net price plus 30% of all parts required for the repair. For V6620, this amount should include the parts allowance for any replaced banjo washers when applicable.

*** The $#.## identified in the "Net Amount" column represents the cost plus 30% for one (1) pint of brake fluid used in conjunction with labor operations.

**** Labor hours required to replace tapered pads, install actuator levers, and adjust park brake cables are included in the standard campaign labor hours (V6620). No addititional time is needed as procedures to perform these repairs are part of the campaign actuator installation procedure.
